Webinar: Chronic Care Management and Depression Screening by WPS

WPS will provide information about Chronic Care Management (CCM) and Depression Screening (DS).  We are responding to a CMS MAC Innovation Challenge to increase the use of these services by our beneficiaries.  Part of our challenge is to promote health equity for all Medicare patients. 

CCM services are non-face-to-face services to increase proactive management of patient’s chronic conditions and other problems.  CCM can help reduce a patient’s need for

• Emergency department visits,
• Hospitalizations, and
• Urgently needed in-person visits to your office. 

CCM pays for providers and their clinical staff to develop and implement a comprehensive care plan for the patient.  This seminar will provide the Medicare rules and regulations.  This will include patient eligibility and requirements for the care plan. 

Medicare allows a yearly preventive depression screening service.  This preventive service can help identify those patients that may benefit from further mental health services.  Data analysis shows patients attempting suicide often saw their physicians within the week and month prior to the attempt.  WPS will provide information on eligible providers and patients and frequency requirements.
